This is a work in progress and I would appreciate comments and feed back.
Applying for a DTS can be a huge step. For some people, it is the first time they are leaving home.
To make the process manageable, both for the applicant and School Leaders I am breaking it down into steps.
- First, let’s just get the applicant’s name and contact information and the school they are looking at.

- We then send the applicant a link to a new Google Form for the school application. Example.
- We also send them their Medical Form to fill out. Example.
- Once we have their reference information we can send an email to each of the referees to fill out the
Using Google Forms.
The following form is an example of how you could use Google Forms to start the Application process for a school.
The applicant has to use a Google Account to be able to use this form.
If they are not already signed in to their Google Account, they will be asked to do so.
This is where you might need to consider if you want this degree of restriction on your online applications and would want to have a PDF as well for people that can not be online or do not what to use a Gooogle Account.
The use of a Google Account makes it possible for someone to return to their application at a later date and complete it.
Options
- Google Account-based Form people fill in and can return to.
- Google Forms without the connection, so if they forget something or make a mistake they have to start over from the beginning again.
- PDF-based form, offline and they email it back to you.
The first two options give us the ability to take an application and automatically create a printable Google Doc from the form information given.
